2019 AND PRIOR YEAR PERFORMANCE REVIEW SYSTEM INFORMATION:

MCAD's annual performance evaluation cycle runs from June 1st through May 31st of the following year. Although performance evaluation is an on-going process, this is your opportunity to formalize feedback for the individuals you lead and to encourage them toward continued growth and development.

You'll find the performance evaluation forms, in both WORD form-format (this is a fillable form) and PDF format (this form must be printed and then filled in), on MCAD's HR Intranet Site under the heading "Forms/Performance Evaluation." These performance evaluation forms are categorized by review type and then job classification.

Review types are:
  • 3-Month Review
  • First Annual Review with associated Self-Assessment Worksheet (Long Form)
  • Subsequent Reviews with associated Self-Assessment Worksheet (Short Form)
Job classifications are:
  • Department Heads/Supervisors
  • Professional/Exempt (salaried employees)
  • Support Staff/Non-Exempt (hourly employees)

The process begins when you ask each of your employees to complete a self-assessment worksheet. Once you receive this information from your employee, you will then select and complete the associated performance evaluation form, integrating the information provided to you on the employee's self-assessment worksheet. You will then obtain the required signatures (1st and 2nd level supervisors) before discussing the performance evaluation with your employee. Completed performance evaluations should be submitted to Human Resources by August 1st each year. You may keep a copy for your reference. Complete performance evaluation instructions can be found on the first page of every form, under the heading "Instructions to Immediate Supervisor."
