ESA Requirements for Students who are Applying for an ESA Accommodation

  1. REQUESTS FOR ESA'S AFTER MOVE-IN MAY BE GRANTED FOR A FUTURE SEMESTER, NOT THE CURRENT ONE.

  2. AN ESA REQUEST DOES NOT EQUAL APPROVAL.

ESA REQUIREMENTS FOR STUDENTS WHO ARE APPLYING FOR AN ESA ACCOMMODATION
 
  1. Send an email to Accessiblity Services (accessibility_services@mcad.edu) with weekday times you have available for a 1 hour virtual or in-person meeting to go over accommodations and ESA policies

  2. Apply for accommodations at accommodations.mcad.edu

  3. Upload or email Accessibility Services an ESA Accommodation Documentation Form or documentation that includes all of the information in this form completed by a health care professional that you see regularly who has recommended an ESA as part of your ongoing treatment plan

  4. Upload or email Accessibility Services a copy of MCAD's ESA Contract that is filled out as much as possible before school starts
    1. If you have roommates then have them send an email to housing@mcad.edu and accessibility_services@mcad.edu stating they are okay living with your ESA
    2. You can skip all signatures but yours in the contract
  5. Upload or email Accessibility Services a picture of your ESA

  6. Upload or email Accessibility Services  current rabies vaccination or other shots required by law in the USA and MN for your ESA if applicable
      1. If this is required by law for your animal and we do not have it when you bring your animal to campus then we will ask you to take your animal off-campus. You can find some information about the laws on MCAD's ESA website. It is your responsibility to know the laws regarding your animal.
  7. Upload or email Accessibility Services a copy of your ESAs Minneapolis pet license information or receipt of payment if applicable
    1. If you have paid for the pet license and it has not arrived yet then we will need a copy of the receipt to show you purchased it.
      1. Once you receive the license then you can upload a scan or picture of the license information at accommodations.mcad.edu.
    2. If a license is required by law for your animal and if we do not have at least a receipt saying you have paid for it when you bring your animal to campus then we will ask you to take your animal off-campus. You can find some information about the laws on MCAD's ESA website. It is your responsibility to know the laws regarding your animal.
  8. You can find the contract and other information on MCAD's ESA website.

HOW TO UPLOAD FILES

If you filled out an accommodation application in the past and need to upload more documents then you may do so following the directions below.

  1. Go to accommodations.mcad.edu
  2. Click on the "Additional Accommodation Request Form" link
  3. Click on submit
  4. Fill out the form
  5. Click submit again to finish the request
  6. The next page will have a place for you to upload additional documentation
