Student FERPA Form instructions
What is FERPA?
For higher education, FERPA, otherwise known as the Family Educational Rights and Privacy Act of 1974, is a federal law about the release of and access to specific student educational records and information. For more information, visit FERPA online.
Students wishing to grant access to MCAD student account information, financial aid, on-campus housing contracts, and/or records, to a parent, relative, or another third party, must submit an online FERPA form, available at myMCAD. ** Students are required to submit a separate, one-time, FERPA form, online, for each requested user.
How do I complete this form?
- Log into myMCAD with your MCAD username and password.
- Click the ‘FERPA FORM’ tab at the top of the page.
- Carefully read the instructions and click the ‘Grant Access,’ link near the bottom of the page.
- Fill out the form in its entirety (3 pages) and click, ‘Submit’.
*DO NOT CLICK THE BACK BUTTON WHILE COMPLETING THE FORM!*
If, for any reason, you need to return to a previous section, please click the “Previous Page” button at the bottom of the FERPA form.
Once you have successfully submitted the online form, you will receive a verification email from the Records Office. ** Please allow 1-3 business days to process your request**
After Records has processed your submitted form, the authorized parent, relative, or other designated user will receive an email notification from the Records Office with instructions for creating their online account, which will grant to access the information you have selected specifically for that person.
Students may revoke FERPA permissions anytime by visiting the Records Office, M-F, 8:30 AM to 5:00 PM in M103, located on the first floor of the Morrison building.
